The running pleated suture technique is designed for wounds with two sides of unequal length. In this example, this side is longer than this side. This technique is executed as follows an anchoring suture can be placed at one pole. This is then tied off, using an instrument tie.
The needle is then reloaded. And again, recall... that this is the wound side with excess. So, on this side, we are going to take widely-spaced and superficial bites. Widely-spaced, from here to here. And superficial, meaning that the needle is only coursing through the superficial dermis.
In contrast, on the contralateral side, we are going to take deep, narrowly-spaced bites. As we continue along the course the wound, again here, widely-spaced and shallow. Here; deep and narrowly-spaced. The wound edges gradually come together. Widely-spaced, shallow.
Deep, narrowly-spaced. Again, widely-spaced, shallow. And deep, narrowly-spaced. And again, widely-spaced, shallow.
And deep and narrowly-spaced. This approach continues along the entire course of the wound, after which point, the suture is tied. Notice here, however, that the wound edges are now well-opposed with minimal excess on either side.
