The splint that we are going to apply today is a posterior splint to the lower extremity. The posterior splint runs from the toes along the plantar aspect behind the calcaneus and then up along the back of the leg to the level of the knee. The purpose for this splint is to immobilize injuries to the foot, to the ankle, and to the distal low leg.
This splint is commonly used for fractures as well as sprained ankles and injuries to the feet. When applying this splint, there are several areas of padding that are of importance-- the calcaneus, the lateral malleolus, and then on the medial side, the medial malleolus. These areas need to be padded because they can cause pressure and pain. In preparation for application of this long posterior splint, I'll have my assistant stand on one side, and I'll be on the opposite side of this patient.
Prior to any splint immobilization, it is important to assess the motor, sensory, and vascular status of this patient's injury. So now I'll have my assistant put her hands at his toes and then one hand under his knee and flex his knee slightly, trying to keep the ankle in a neutral position. The splint will go from the toes here, and then I'll have my assistant hold on the other side of the splint.
Again, extra padding may be needed around the calcaneus or on the medial malleolus or the corresponding lateral malleolus as well. We'll use the ACE wraps to help hold this splint into place. And then once the splint is in place, we will mold it in a neutral position and allow the splint material to harden. After the splint has become rigid, we'll again assess the sensory and the vascular status of this extremity.
Even though this splint will dry in a rigid fashion, this splint is not meant to allow the patient to weight bear. So crutches or a walker or a wheelchair will be important to use for this type of injury.
