>> In medical settings, patients often present with more prominent psychological and emotional complaints than they do physical complaints. In that case, you proceed similarly except you focus on the psychological and emotional complaints, which often reflect an underlying mental health disorder. You're going to see more of the clinician centered interview to demonstrate how one obtains key information necessary in behavioral health patients.
KNOCKING: KNOCKING : >> Good morning. Hello. >> Hi. >> Hi. I'm Dr. Hasanain. I'm one of the internal medicine physicians here in the clinic and I'll be taking care of you. Nice to meet you. >> Hi. And you as well. >> So did you find everything OK this morning on your way to the clinic? >> Yes. I sure did.
KNOCKING: >> Wonderful. So we kind of have 15 to 20 minutes to go over any of your concerns this morning. Could you tell me more about what has been going on? >> Well, my sister's been bugging me to come in and get checked out because she's worried there's something really wrong with me. >> OK. >> I was anemic about 10 years ago and she said that a lot of what I'm going through right now is the same as what I was going through when I was anemic. >> OK.
KNOCKING: >> So she thinks maybe, you know, that might be what's going on. >> We're going to address the fact that you might have been feeling tired lately. Anything else that you would like to address today? >> I -- I guess I just am concerned about the fact that I just seem to be so tired all the time. >> OK. OK. >> Even though I sleep a lot during the day. So that -- that does concern me. >> Could you tell me more about that you've been tired?
KNOCKING: >> Well, I seem to sleep and nap a lot throughout the day. I just don't have any energy. I feel fatigued all the time. >> OK. >> And I don't really have an appetite. So just not sure what's all going on. >> OK. How has that affected your life? >> Well, I know I need to get up and look for another job, but I just can't seem to get up and get going, get motivated to go out and look for a job.
KNOCKING: So that's really been a big hindrance for me. >> Tell me more about your job. >> Well, I got fired last August. My attendance record was not very great and I know my work performance just wasn't what it had always been before. So they let me go in August and I've been on unemployment since then, but it runs out next month and I'm really concerned about money. I think that's why my sister's really been trying to push me out to go look for a job, but I just have no energy.
KNOCKING: I just am not motivated to go out and look for another job. And I really just don't know who would hire me at this age and having been fired. I guess I'm just not motivated to go look. >> Could you tell me more about how -- how that makes you feel? >> Well, it's -- it's embarrassing because I did get fired. It's not something I really would have expected to happen. And I feel really guilty because I've been sleeping on my sister's couch since January because I had to give up my apartment.
KNOCKING: I just couldn't afford it anymore. And so I'm imposing on them. It's also irritating because her two teenagers are kind of, um -- they're loud and they're rude and it's not a very pleasant environment for me, but, at the same time, I'm just grateful that I have a roof over my had right now. >> Got ya. >> So a lot of different emotions all at the same time. >> Got ya. Well, I can only imagine how that would make you feel.
KNOCKING: You seem to have -- to be having a lot on your own plate right now. You said you're -- you felt embarrassed when you were fired. You're feeling guilty. >> I guess I'm just not feeling really good about where I am at this point in my life. I don't have much strength. >> Well, actually that's why we're here. We're going to be guiding throughout the way and we're going to be taking care of you. >> OK. >> So I'd like to switch gears right now and kind of ask you some questions.
KNOCKING: Have you been feeling more depressed lately? >> I -- I would say yes. I have experienced some bouts of depression in the past. >> OK. >> And I guess you could say what I'm feeling right now feels like that again. >> Any problems with appetite at all? >> I don't have much have an appetite right now at all. >> OK. Have you had any thoughts of harming yourself at all? >> No. It hasn't gotten to that point at all.
KNOCKING: >> Any thoughts of harming yourself that you've had in the past? >> No. >> OK. And you said you've been having episodes of depression in the past. Did you seek medical advice? >> Yes. They put me on medication and I was seeing a therapist. And actually both of those helped at that time, but it's been several years since I've gone through anything like that. >> And do you have any family history of any depression? >> Yes. My mother and my other sister both have suffered from depression as well.
KNOCKING: >> OK. Do you have a support system? >> My sister to some extent, but, you know, like I said, she's really been pushing me to get out.
