Hi I'm Dr Dipit Sahu. Welcome to my work. So I'm sure the surgeon. And today in this video, in this presentation, I'll be showing you and discussing a case of supra scapula nerve compression at the supra scapula notch by a cyst. And we will be decompressing the nerve by releasing the transfer scapular ligament and decompressing the cyst.
So let's have a look at the patient profile. This is a 30-year-old guy with severe, persistent pain and weakness of external rotation. MRI shows a cyst on the supra-scapular notch, and the treatment is vital to cope with decompression at the suprascapular muscle. This animation is to demonstrate how we call medially along the supra scanatic muscle and reach the supra scaplan notch and then release the transverse scapular ligament
thus decompressing the supra scaplalar nerve as shown here. The patient is placed in wheelchair position. Two lateral portals are utilized for the procedure, shaver and arthoplasty used to scope out in order to do lateral progress. Dissection is then carried out posterior to the corraco claviclar ligaments immediately to transfer.
Scapular ligament is isolated at the supra-scapula notch. The transfer scapular ligament is visualized clearly and tissue that cleared medial and lateral to the ligament. Superial medial portal is then made and a blunt probe inserted.
This probe helps to clear up tissues. A cyst is also noted just medial to the ligament. This cyst is punctured and decompressed with the help of the blunt crowbar. The gelatinous fluid from the cyst can then be visualized escaping from the cyst. The ligament is visualized again and tissues are separated medially and laterally at the notch. This ligament is then transected with the help of basket punch inserted from the superiormedial portal .
We know the ligament. The nerve is visualized crossing the notch. So this is how supra-scapula nerve is decompressed. Thank you.
