The running locking suture is very similar to the simple running suture. As always, an anchoring suture is first placed. Secure it using an instrument tie. For demonstration purposes, again, I'm going to leave the tail in place for the anchoring suture. Needle is then reloaded.
Simple interrupted suture is then placed. At this point, the needle driver is brought from around the loop that was newly created, and grasps the needle, pulling laterally. This leads to a secondary line of suture that runs parallel to the incision line. The process is then repeated along the length of the wound.
When one pulls laterally, the secondary line of suture that I mentioned will be visible parallel to the incision line. What this does is effectively lock each loop of suture in place. Now because I'm using a skin substitute here along with a monofilament suture, the locking effect is slightly less pronounced than it would be in vivo. Again, I'm sliding in from the side, pushing under so that I grasp the loop, grasping the needle, and then pulling laterally.
You'll notice, again, that that loop of suture continues. Again, moving through that loop and then the needle is grasped. This is essentially a shortcut approach to locking that suture. Once the end of the suture is reached, one can either go ahead and perform an anchoring suture by taking a backhanded throw or a forehanded throw depending on the preference.
