OK. I've just mentioned classification of herniae. This can be done in a number of ways. You can classify it by the type of tissue protruding, or by the site of the hernia. One of the best ways is a classification that divides them up into three categories-- a structural category.
And this involves muscle hernia or disc hernia. Also, there are cranial hernias, where it can be herniation of the cerebellar tonsils or, in fact, herniation of cerebral lobes, either through a fracture or through a fractured tegmen tympani, allowing the cerebrum to displace inferiorly into the external acoustic meatus, so that you could actually hang earrings off your parietal lobe.
And the third type is abdominal, which are internal or external. The internal abdominal hernia, good example, is the diaphragmatic hernia. And the external, though, is the classic one that most of us deal with in emergency, in surgery, when we see patients in rooms. Now, the internal hernia, surprisingly, is usually clinically obvious by the history and the examination, such as being hit by a truck or hearing bowel sounds within the chest is fairly much a big giveaway.
It's the external that tends to cause some difficulties in deciding what type of external abdominal hernia it is, and in fact, deciding is it an abdominal hernia at all? Is it some other type of inguinoscrotal lump? And it's important to be able to assess inguinoscrotal lumps well.
