To assess active thoracic lumbar flexion, ask the patient to bend forward, allowing the arms to hang down. Observe the quality of the movement, as well as the extent of the motion. Once the patient has returned to the upright position, assess extension by asking the patient to bend backward with hands on the hips. For lateral flexion, ask the patient to bend to the side, letting the hand slide down the side of the thigh.
Have the patient repeat this motion on the other side. For trunk rotation, ask the patient to twist to one side at the waist, and then to the other side. If the patient's pelvis and hips are allowed to turn as well, the result will be an indication of overall body rotation rather than trunk rotation. Remember, for all motions, watch for any asymmetries, aberrant movement patterns, or indications of pain.
