The buried horizontal mattress suture is performed by reflecting the skin back, inserting the needle in the direction parallel to the incised wound edge and then, having it exit just distal to that pulling the needle through. The needle is then reloaded in a backhand fashion in order to prepare for repeating a mirror image moving in the opposite direction.
Here, again, the skin is reflected back, the needle is inserted through the underside of the incised dermis and then passes through. Gently pull through and then the suture material is then tied using an instrument tie. Notice the dramatic wound eversion that is seen with this technique.
