The larynx and the trachea are identified. The percutaneous tracheostomy tube will be inserted in the trachea inferior to the cricoid ring.
The percutaneous tracheostomy kit includes a number of syringes, needles, a guidewire, dilators, and the tracheostomy tube. The skin at the incision site is anesthetized with infiltration of local anesthetic.
The small vertical or horizontal midline incision is made over the trachea. The subcutaneous tissues are spread using blunt dissection down to the tracheal surface.
The procedure is performed with the assistance of bronchoscopic visualization from within the trachea. A needle is used to puncture the trachea. Bronchoscopy is used to verify intratracheal position of the needle.
Air is aspirated from the trachea. A guidewire is passed through the needle in the Seldinger technique.
A series of dilators are subsequently passed over the wire. This bronchoscopic view shows the guidewire and the trachea and passage of a dilator over the guidewire.
The tracheostomy tube is passed over a large dilator and the guidewire into the trachea.
This final view shows the appliance in the lumen of the trachea.
