FEMALE DOCTOR: Position the patient semi-sitting, with the head rotated to the contralateral side, to expose the posterior triangle of the neck. Identify the external landmarks, the sternocleidomastoid and the clavicle. Asking the patient to lift the head will facilitate the identification of the posterior border of the muscle, when difficult to locate. Place the transducer in a transverse orientation over the lateral aspect of the neck, at the midpoint of the sternocleidomastoid muscle.
FEMALE DOCTOR: Slide the transducer posteriorly until the tapering posterior edge of the sternocleidomastoid is positioned in the middle of the screen. Sliding in a caudal and cranial orientation will help to identify the superficial branches of the cervical plexus, as a small collection of hypoechoic nodules traveling between the anterior scalene and the sternocleidomastoid.
FEMALE DOCTOR: For the intermediate approach, insert the needle in-plane at the level of the transverse process of C4, locating the needle tip between the prevertebral fascia and the investing layer of the deep cervical fascia. For the superficial approach, inject the local anesthetic subcutaneously at the level of C6, posterior to the sternocleidomastoid.
