The lattice stitch is performed by first building the lattice. Essentially, this is done by taking a bite parallel to the wound on one side. Pulling suture material through. Going ahead and tying. And then going ahead and trimming the edges.
This lattice is then built on the contralateral side as well. What we are doing here is building a framework over which we can place the actual suture. So again, a bite is taken parallel to the wound edge. The suture material is tied off. And the edges are trimmed. At this point the actual lattice suture is placed.
And again, this is designed for atrophic skin. There are two ways to approach this. One would be to take a full bite of skin including the lattice within it at this point. Thus entering just lateral to the first suture and exiting just lateral to the second. The other approach is to take an actual bite of the suture. So just gently take the needle, place it underneath the stitch that we just placed and underneath the other one.
And then go ahead and tie off. You'll notice here that the wound edges are pulled together and that the two lattice stitches on the side are pulled together. Instead though of having a single point on either side of the wound where there is tension, this tension is distributed over the four points where these lattice stitches were placed. Several variations of this approach are possible as well.
