When performing the butterfly suture, the needle driver may be held like a pencil. Skin is reflected back. As you would with a horizontal mattress suture, the needle inserts set-back from the incised wound edge. The needle angles outwards, and then exits.
The needle is then passed through the skin and reloaded, often in a backhand fashion, depending on preference. And, again, on the contralateral side, skin is reflected back, the needle is inserted, parallel to incised wound edge, taking a bite that angles outwards, and passed through.
Note that this technique is classically described as being performed on a beveled surface, with the initial needle entry and exit points potentially at the base of the bevel, rather than set-back from the incised wound edge. Notice, again, the dramatic wound eversion coupled with the excellent wound edge approximation, seen with this technique.
