The percutaneous vertical mattress suture. The skin is reflected back, although note that if you can't visualize the undersurface of the skin this technique may be appropriate as well. The needle is inserted up and through the skin. The needle is then reloaded in a backhand fashion.
Note that when that needle entered the undersurface of the skin, it simply moved straight up through the dermis, exiting the epidermis. The skin is then grasped gently. The needle is reinserted just medial to its exit point, passing superficially, and exiting at the incised wound edge. The needle is then again reloaded, again in a backhand fashion. The skin is grasped on the contralateral side.
Again, the needle passes superficially, exiting the skin again. The needle is reloaded in a standard fashion. And the needle is inserted just lateral to its exit point, running deep, and exiting the undersurface of the dermis. The suture material may then be pulled through, and an instrument tie is performed.
Notice the excellent wound edge apposition, coupled with the excellent wound eversion seen with this technique. Also notice that when performed properly, this technique often does not leave visible dimples or visible suture material on either end.
