The purse-string suture is an approach designed for a fairly large defect. Needle is inserted running parallel to the edge of the wound and then exits further along. This approach continues running along the edge of the wound. Notice that some suture material can be pulled through with each throw, and, if necessary, we can shift to a backhand technique.
Suture material is pulled through. And, again, this approach continues with each bite taken running parallel to the circumference of the wound.
These bites continue along the periphery until the suture material exits close to its original entry point. At this point, suture can be pulled, and you will notice the purse-string effect that this has on the tissue. This puckering of the tissue is a natural response to the tension vectors of this approach. At this point, the suture may be tied.
