DOCTOR: Let's review the axillary brachial plexus block. Here's a very good example of how the axillary brachial plexus anatomy substantially varies. Here, we can see, we have two arteries, the artery above is the axillary artery, whereas the axillary below could be lateral thoracic artery, it could be humeral artery or thoracoacromial artery. it could be humeral artery or thoracoacromial artery. In this particular example, we see that the median and the ulnar nerve are below the artery, actually.
DOCTOR: An injection of 8 mL of local anesthetic is sufficient to block the ulnar and the median nerve. But now we're going to need yet another injection in order to block the radial nerve, which is underneath these two nerves. So the needle is now redirected posteriorly to pass over what we believe is the humeral artery, and then placed just above the conjoint tendon. It is important for the needle not to penetrate the conjoint tendon, because the injection will occur outside of the axillary brachial plexus.
DOCTOR: But here, another 8 mL of local anesthetic is injected. And here we can see how there are two compartments, compartment with the ulnar and the median nerve and compartment with the radial nerve, which we have just blocked with another 8 mL of local anesthetic.
