ORTHOM8: Welcome to OrthoM8. Making learning Orthomatic by Harry Benjamin-Laing. Today's topic is the differential diagnosis of acute hip pain in children. This includes transient synovitis, septic arthritis, proximal femoral osteomyelitis, slipped capital femoral epiphysis and Perthes disease. Transient synovitis is defined as inflammation of the synovium of the hip.
ORTHOM8: It's the most common cause of an acute, painful hip in a child. It commonly occurs between the ages of 4 and 8. It is twice as likely in males than in females. I've had a history of an upper respiratory tract infection a few days ago and the pain is improving as the day goes on. My temperature is mildly raised or normal and my X-ray is normal.
ORTHOM8: My CRP is less than 20. Remember, transient synovitis is a diagnosis of exclusion and can be treated with non-steroidals and close observation.
