The percutaneous purse-string closure is performed by taking the needle coming up through the underside of the dermis, percutaneously exiting the skin, reloading the needle, and then inserting the needle again, either through the same hole that it exited, or just adjacent to it. Repeated bites in this fashion are taken moving around the periphery of the wound.
Depending on the gauge of suture material used, the size of the wound, and the tension across the wound, the surgeon may wish to perform this technique fairly set back from the incised wound edge.
This technique continues around the entire periphery of the wound. And, again, depending on the size of the needle, bite size, of course, can be varied.
Once the end of the wound, where the suture began, is reached, suture material is pulled through, and the suture is pulled taut. Note that here, I began on the undersurface of the wound, so I will end on the undersurface of the wound. Though sometimes when using this percutaneous approach, individuals may prefer to start and finish on the outside of the wound.
Suture material is gently pulled through, leading to an excellent purse-string closure. And once the suture is tied, this leads to some dramatic buckling at the center of the wound, but also dramatic reduction in the wound size.
