The set-back buried dermal suture is a workhorse technique. It is performed by reflecting back one edge of the wound. After effective undermining has been performed, the underside of the dermis is exposed. The needle is inserted at 90 degrees into the underside of the dermis, and then exits, again, at the underside of the incised dermis.
Note that the needle does not exit at the incised wound edge. Needle is re-grasped and loaded immediately, and then the same procedure is repeated on the contralateral side. Wound is reflected back, needle is inserted through the underside of the dermis, now heading outwards more distally, exits again further set-back on the underside of the dermis, re-grasped, and pulled through. Suture material is then pulled through and tied.
Notice the excellent wound eversion and wound edge approximation seen with this technique.
