OK, in knee arthrocentesis, the suprapatellar pouch I never use because in 12% of people, there's a plica. So even though there's an effusion in the knee, if you use this suprapatellar pouch approach, you're going to not get any fluid because of that plica. So what I use is the infrapatellar approach developed by Hollander. And this is the tibial tuberosity, the patella.
Tibial tuberosity and patella, patellar tendon. Next to the patellar tendon, you go right above the tibial tuberosity. You go right into the joint, all the way through. And that's the approach that I use. You can go in either the medial or the lateral side. And you aim the tip of the needle to an artificial space that is between the two femoral condyles.
Right in there, you see the tip is aimed toward those two areas.
