To evaluate a patient for active neck range of motion, make sure the patient is in neutral postural alignment. It is helpful to demonstrate the motions yourself. For capital flexion, ask the patient to tip the head forward, as if to touch the chin to the neck in a nodding motion. Then, have the patient tip the chin up, looking up for capital extension. For cervical flexion and extension, ask the patient to bend the neck forward to look down at the floor, and then to extend the neck to look up at the ceiling.
To assess cervical lateral flexion, ask the patient to bend the head to the right side, bringing the ear to the shoulder and then to the left. For neck rotation, ask the patient to turn the head to one side, as if to look over the shoulder and then turn in the other direction. During all motions, watch the patient carefully for any asymmetries, aberrant movement patterns, or indications of pain.
