So hello, friends. Today we are going to show you a surgical technique in which we will put k-wire for a pseudo bennett fracture. I have Dr. Ghaffar Khan, who is my colleague. So he will be doing this operation for you. So looking at this X ray, if you see this gentleman, he has got an extra articular fracture, which we also call a pseudo Bennett fracture. And other type of fracture that we see in this region is intra articular fracture, which is a Bennett or a complete intra articular fracture.
Which is another kind of renata fracture. So this is one of the commonest of fracture. And all we need to do is to reduce it and secure it with one or 2K wires. So our Dr. Ghaffar Khan is going to take the lead and he will operate. I'll take you to take you through how to do this surgery safely. So I usually do this operation under general anesthesia. However, if you haven't got anaesthetist, you can give a good regional block.
Then this operation can also be done under regional anesthesia. However, on this occasion we are doing it under general anesthesia. The patient is lying supine and we don't use tourniquet because we're not going to make any big incision. So all this needs is a reduction of the fracture and passing off two k-wires in order to hold the fracture. [That's a good thing. ] So when you are passing wire in this region, I think Doc Ghaffar has passed the first k-wire and all you have to be careful is about the epil because it is very close and you get terminal sensory branches of the radial nerve.
So once we pass the first wire, we check it in the CR. So if you look at the position of the wire, it is gone through the first metacarpal and is going into the carpal bones. And in order to reduce this fracture, all you need is to have a longitudinal, longitudinal traction so as to reduce the fracture by the government of Texas [?].
So now we have passed our second wire and if you look at the CR now, the fracture is nicely reduced and last thing I do is to do a dynamic screen in order to assess the stability. So all we are going to do now is to bend the wires and we will put immobilize the thumb for three weeks. After three weeks, we'll take the wire off and start mobilization.
