The pulley suture is used typically intraoperatively in order to pull tissue together that's under significant tension. Start out with a far bite, reload the needle. Instead of taking the next bite in line with the first bite, we can move a little bit distally and take a near bite. Suture material is pulled through and another near bite is taken on the contralateral side.
This technique ends with another far bite directly across from the initial suture placement. Suture material is pulled through and depending on the degree of tension, either the ends of the suture can be pulled taut, and secured with the aid of hemostats, or the suture material can even be tied. Typically, however, because this approach places a great deal of tension across the epidermis, it is used intraoperatively for tissue expansion, rather than as a permanently placed suture.
