Hello, so today I'm going to demonstrate you a clinical test, which is called quadriceps active test. This is a test which is used to see if PCL is torn or not. PCL is a very strong ligament in the knee, which is called posterior cruciate ligament. And I use this test routinely in my clinic in order to diagnose PCL insufficiency.
So when you are to do this test, you get the patient supine on the table. I get them to bend the knee up to roughly around 90 degrees. And first thing you will see is the side you can see there is a side here. If you see the other side, it looks pretty normal to me. So if you see a side, then in your mind, you must be thinking that there might be damage to pursue a cruciate ligament.
Now how do you go about this active test? So in order to do this test, first thing is that you put one hand here and then you ask patient to kick, OK, just kick and let go, and kick, as it's sticking, you can see, due to function of quadriceps it is pushing or it is pulling the tibia forwards. And this is the positive test. This is suggestive of a PCL injury. So just to do it again, relax, push, relax, push.
So every time is pushing his activity in the quadriceps, and it is reducing the posterior side and it's pulling the trigger forward. This is classic and positive for the subtractive test, which is suggestive of torn PCL. So for you, this was a demonstration of quadriceps active test for PCL injury, I would recommend wherever you are suspecting PCL injury do this test. And if PCL is, torn, every time you will find this test to be positive.
