DR. SONG: This video shows the peroral endoscopic tumorectomy or POET procedure of the mid-esophageal subepithelial lesion. Approximately 5 centimeters above the lesion, a longitudinal mucosal incision is performed following submucosal fluid injection. The endoscope then enters into the submucosal space through the mucosal incision.
DR. SONG: Dissection of the submucosal space is carried out using an electrosurgical knife with intermittent fluid injection. Once the lesion is reached, further dissection is carried out to free its attachment to the muscularis propria. Careful dissection is performed in order to preserve the pseudocapsule of the lesion.
DR. SONG: This lesion originated from the muscularis propria as seen on the prior endoscopic ultrasound procedure. Intermittently, the endoscope will exit the submucosal tunnel in order to inspect the mucosal layer to assess for any thermal injury. Once only a small pedicle attachment remains, the lesion will be resected en bloc using an electrosurgical snare.
DR. SONG: The resected specimen will then be grasped with a net and retrieved through the submucosal tunnel to exit the mouth. This 2.5 centimeter lesion proved to be a leiomyoma. The resection site is inspected following tumor removal.
DR. SONG: The preserved adventitia can be appreciated at the 12 o'clock position. The mucosa is inspected at the tumor resection site to assess for any thermal injury. In this case, focal injury was apparent, which was treated with placement of endoscopic clips to prevent delayed through-and-through perforation.
DR. SONG: The mucosal incision site was closed using an endoscopic suturing device, which enables placement of sutures in an interrupted or running fashion. Here, the mucosal incision site was closed with placement of two stitches in running fashion. The patient tolerated the procedure well and discharged from hospital the next day.
