The running horizontal mattress suture is placed by first tying an anchoring suture at one pole of the wound and gently tying that off. At this point, the needle is reloaded, and the horizontal mattress component is begun.
The needle is inserted at 90 degrees exiting on the contralateral side. The needle is passed through, reloaded with a backhand fashion, exiting and now entering on the same side, traversing across the incised wound edge, and, again, reloaded, now in a forehand fashion.
Again, the needle enters proximal relative to the surgeon from where it entered, but on the ipsilateral side, passing across the wound. And, again, you can notice the dramatic wound eversion that we're seeing with this technique. And, again, needle inserts proximal to its exit point on the ipsilateral side exiting on a contralateral side of the wound edge.
This technique continues along the course of the wound until the end of the wound is reached, at which point an instrument tie is performed and the suture material is simply tied off. This leads to an everted wound edge, which is generally desirable, and reasonably good wound edge approximation.
