Long arm posterior splint is used to immobilize injuries to the forearm, to the elbow, to the supercondylar area of the elbow, and in some cases, a distal humerus type fracture. It is important when you apply the splint to watch and protect the bony prominences of the ulnar styloid process, the olecranon, and both the medial and lateral epicondyle of the distal humerus.
This extra padding will also help to prevent sores and pressure. It is important when you put the splint on that you immobilize the wrist to help reduce pain at the elbow and movement. Also make sure that when you apply the splint, you check the vascular and sensory status of this extremity before and after you apply the splint. I've moved to the opposite side of the patient in preparation for splint application.
What I will do is have my assistant to put one hand on the patient's hand, and another hand underneath their elbow to help support. I've premeasured the splints to apply for this patient's arm. The splint will start at the hand, and I will have my assistant hold that. And then, also, push the splint up along the back of the arm and have my assistant hold that splint as well. I'll start at the wrist with the ACE wraps and include the hand to help cut down on swelling that may get pushed into the hand with this type of splint.
Can you go ahead and hold that? And then continue to work the splint and ACE wrap up the arm. Once the ACE wraps have been applied, then we will hold the splint in a position of function, with the elbow bent about 90 degrees for comfort and for support for the elbow and forearm region. Once the splint has dried and is in an appropriate position, then we will go ahead and check the motor and sensory function of this extremity to make sure there are no changes pre- and post-immobilization.
