For the running percutaneous buried vertical mattress suture, you start by placing a percutaneous buried vertical mattress suture at the wound apex, inserting it from the underside of the dermis, exiting through the skin, pulling the suture through and reloading the needle in a backhanded fashion. The needle then enters just medial to its exit point, passes superficially through the skin, exiting at the incised wound edge. The needle is then immediately reloaded, again in a backhand fashion.
It is inserted again superficially through the contralateral wound edge, again, exiting up through the skin. Again, suture material is gently pulled through, and the needle is reloaded in a standard fashion. At this point, the needle is inserted just lateral to its exit point, and it exits in the open wound space.
An instrument tie is then performed to anchor the suture in place. And notice the pronounced dimpling, noticeable on both sides, with the placement of the suture. At this point, the anchoring suture has been placed, and we are ready to place a series of running percutaneous buried vertical mattress sutures. This is done by simply continuing the process along the course of the wound.
Moving proximally relative to the surgeon, the needle is inserted through the undersurface of the undermined dermis, piercing up through the skin. The needle is then reloaded in a backhand fashion as suture material is pulled through. The needle is inserted just medial to its exit point, coursing superficially through the skin, exiting at the incised wound edge. The needle is then immediately reloaded.
Suture material is pulled through, and, again, the needle is inserted on the contralateral side through the incised wound edge, coursing superficially through the skin, and exiting. The needle is then reloaded in a standard fashion, inserted just lateral to its exit point, and the process continues. This will continue, working in pairs along the course of the wound, so that a running line of percutaneous buried vertical mattress sutures will be placed.
Once the end of the wound is reached, the suture material may be tied off, and the knot is buried.
