The hybrid mattress tip stitch was paradoxically originally described in the literature as a vertical mattress tip stitch. As will be clear from the demonstration, conceptualizing this as a hybrid mattress tip stitch may it make it somewhat easier to learn. Needle is inserted at 90 degrees, relatively far from the wound edge, and, again, this is proximal to the actual tip. Needle then exits far.
Some suture material is pulled through, and the needle is reloaded in a backhand fashion. At this point, the needle is going to be inserted distal relative to the surgeon, so that it is closer to the tip and closer to the incision line. Note the similarity between this approach and a typical hybrid mattress suture.
A bite of the dermis in the tip of course is taken. And then the needle reenters on the contralateral side, again running superficially, and exiting near to the incised edge. At this point, some suture material can be gently pulled through. Of course, we don't want too much tension across this wound.
And the suture material is tied off. You will notice that this leads to an excellent three-point approximation, as well as excellent eversion of the wound, and you'll also notice the chevron formation, which is similar to what is seen with the standard hybrid mattress suture.
