The stacked backing out subcuticular suture, also known as the "super loop suture," is the combination of a deeply placed fascial or deep dermal running suture, followed by a more superficially placed standard running subcuticular suture. We enter at the far apex of the wound from the outside of the wound, entering at the interior of the wound.
And a series of deep fascial or deep dermal bites are taken, moving along the length of the wound. As with a standard running subcuticular suture, we can introduce a small amount of backtracking with each bite, in order to reduce the risk of pleating of the tissue.
Once the end of the wound is reached, suture material is pulled through, leaving a substantial tail. The needle is inserted from the interior of the wound, exiting lateral to the apex, and suture material, again, is pulled through. The needle is then reloaded in a backhand fashion, and now a series of superficially placed, or standard type subcuticular sutures, are placed, but now moving in the opposite direction.
Needle enters from, again, outside of the wound, exits into the interior of the wound. And now, again, a series of superficially placed running subcuticular sutures are placed. This process continues along the entire surface of the wound until the end is reached.
Once the end of the wound is reached, the needle, again, is inserted from the interior, exiting lateral to the apex, adjacent to its entry point. And at this point, the suture material can be gently tied off. This procedure leads to a combination of a deeply placed set and then a superficially placed set of sutures, which, again, can of course be very helpful in some situations.
