For the running percutaneous set-back dermal suture, this begins by anchoring a percutaneous set-back dermal suture in the beginning. Forehand throw followed by backhanded throw. You can combine those two steps.
Suture material is gently pulled through. And, again, we then reinsert deep, exiting at the interior of the wound. Instrument tie, of course, is performed. Since I'm using a monofilament suture, I am using forethrows to lock the suture.
Loose end is trimmed, and the needle is reloaded. At this point, a series of running percutaneous set-back dermal sutures are placed. Skin's reflected gently, if possible, but if it's not possible, that is all right with this technique. Needle is inserted from underneath, exiting directly through the skin.
Suture material is pulled through. Needle is reloaded in a backhand fashion, inserted just medial to its exit point. We can combine that step with the following step by pushing the needle through to the contralateral side. Being careful not to grasp the tip of the needle. Pulling through, again, reloading the needle in a standard fashion, inserting lateral to its exit point, exiting in the open wound space.
And a series of these sutures is then repeated. Each time moving a little bit more proximally relative to the surgeon. Once the end of the wound is reached, the suture material is tied off, and the knot is buried.
