The stacked double purse-string closure is designed for larger wounds where a standard purse-string closure may not be adequate. Here the wound edge is reflected back and a purse-string closure is performed, but now very far set back from the incised wound edge. This closure continues around the periphery of the entire wound.
An abbreviated version of this closure is performed here simply for demonstration purposes. Again, each bite is taken very far set back from the incised wound edge. Suture material is pulled through.
And a partial closure is affected, if possible. Notice that the size of the defect, while not obviated entirely, does decrease significantly.
At this point, the ends of the suture may be gently trimmed and the second round of purse-string sutures will be placed. The second round of purse-string sutures is placed much closer to the incised wound edge in order to lead to excellent epidermal approximation, or at least epidermal approximation that is far better than it would be seen by placing a single layer of purse-string sutures alone.
Again, an abbreviated version of this technique is performed here simply for demonstration purposes. Once the end is reached, again, suture material is pulled through and a closure is performed using an instrument tie.
