A Radial Gutter Splint is commonly used to immobilize injuries to the distal radius, to the index and long finger metacarpals, as well as the index and long fingers themselves. Also, used sometimes for a fracture in the base of the thumb. The split is measured from the tip of the fingers to the base of the proximal forearm. Areas to concentrate on for extra padding would be the distal radius at the radial styloid process, the metacarpal joints at both the index and the long finger, and then also at the base of the thumb.
Be sure to check the neurovascular and the sensory status of the extremity that you're going to splint pre- and post-immobilization. After we've pre-measured our splints we apply the cast padding to the splint, then we're ready to apply our splint. We'll go ahead and apply this so that it wraps around his thumb, and then also make sure that the splint comes out over the index and long fingers.
Sometimes people will include the ring finger as well. Again, the splint is applied and contoured to the affected extremity, and then an Ace bandage is used to help apply it. Again, it's important to include the hand in the Ace wrap for compression. This will help to reduce edema in the hand and make it more comfortable for the patient.
Once the Ace wrap has been applied and the splint is set up and is in proper position, it's important to place this radial gutter splint, the hand in a position of function. And so therefore the hand would be positioned in a slight bit of flexion at the MCP joints here, with a little bit of extension at the wrist joint.
