Posterior labral repair using a percutaneous knotless mattress suture. In this right shoulder in the lateral decubitas position, the posterior labral tear can be seen. An anterior portal is created and using a switching stick, the camera reversed to reveal the posterior tear. A spinal needle is used to plan the accessory portal and suture position.
A shaver is used to debride the rough edges of the labrum and to prepare the glenoid. Insert an 18 gauge spinal needle through the posterior labrum and introduce the fiber stick suture into the joint. This can then be retrieved via the accessory portal.
Withdraw the needle through the labrum, but not the capsule, and then re pass it through the labrum to create a mattress suture. Withdraw the needle slightly to create a loop and then take the second limb out through the accessory portal, allowing it to slide.
Both suture limbs then need to be withdrawn through the accessory portal before a 2.9 millimeter pilot hole is drilled. A 2.9 millimetre bio-composite push lock anchor can then be railroaded down the sutures and driven home into the pilot hole.
The suture ends are then cut flush and the process can be repeated with a second suture.
Both suture limbs are then retrieved through the accessory portal. The glenoid is drilled and a 2.9 millimetre bio-composite push lock anchor run over the sutures to be inserted easily.
Once the suture tails have been cut flush, the repair is completed.
