The horizontal mattress suture is a frequently used suture. It's actually quite simple to place. Needle is inserted at 90 degrees perpendicular to the skin, exits on the contralateral side. At this point, the needle is reloaded in a backhand fashion. Some suture material can be gently pulled through. The needle is inserted again, on the ipsilateral side at 90 degrees, exiting back on the same side where the needle first entered the skin.
Suture material is pulled through and an instrument tie is performed. You can already see the marked wound eversion that is classic for this approach. Ends of suture are trimmed. And, again, note the excellent coaptation of the wound edges, coupled with the excellent wound eversion.
