The running alternating simple and vertical mattress suture is placed in a similar fashion to the running vertical mattress suture, except that alternating simple throws are placed. An anchoring suture is placed. In the original description of this technique, the anchoring suture is simply a simple interrupted suture. A knot is tied.
The needle is reloaded and is inserted close to the knot, traveling on a diagonal towards the surgeon, exiting far from the wound edge. The vertical mattress throws here are placed in a far-near, near-far fashion, thus this was the far throw. We are then going to enter near, and exit near, on the contralateral side.
We reload the needle driver, and enter far. Again, we are going to move on a diagonal towards the surgeon at this point. Reload. Now, instead of taking another vertical mattress bite, instead the needle is simply reinserted on the contralateral wound edge, in line with the exit point, and, again, a diagonally placed suture is performed.
Thus, a simple running suture is placed in between the vertical mattress sutures. The technique then continues moving along the course of the wound. This is treated as the far exit point, thus we have a near entry point followed by a near exit point, and, again, a far entry point. This approach continues until the end of the wound is reached, and at that point an instrument tie is performed.
