This video illustrates stent fixation in a patient who underwent temporary stent placement for an esophago-gastric anastomotic stricture, refractory to balloon dilation. Seen here is the anastomotic stricture that did not allow passage of the endoscope. A guide wire was inserted through the stricture into the stomach remnant under fluoroscopy. Over the wire, a fully covered stent was inserted across the anastomotic stricture and successfully deployed.
To prevent its migration, an endoscopic suturing device was used to anchor the stent to the esophageal wall. The system consists of a curved needle with a detachable tip that carries a polypropylene suture. The handle component of the device actuates needle transfer and movement of a suture arm, enabling passage and exit of a suture through the stent as well as tissue.
The instrument is capable of deploying multiple interrupted or running stitches without device removal. In the case of stent fixation, three to four stitches can be placed along the circumference of a stent. These stitches can be removed when needed using dedicated endoscopic scissors or argon plasma coagulation to section the sutures.
As shown here, a suture cinching tool is used to tighten and secure the deployed suture. A total of three stitches were placed in this case to secure the stent and prevent its migration.
